Symptoms

Order creation fails in OSM due to ORA-00001: unique constraint (OSM.XPKOM_ORDER_FLOW_COORDINATOR) violated. Issue is seen in OSM clustered environment during high load and/or during cluster resize (i.e. one or more managed servers joining/leaving cluster).
