Symptoms
On : 12.2.5.1 version, Upgrade
When attempting to upgrade Demantra from 12.2.4 to 12.2.5.1,
the following error occurs.

Errors were encountered during the execution of
"Apply Grants" process:
* ORA-46212: XS entity with this name already exists.
* ORA-06512: at line 579
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Download <Patch.21951481> to upgrade to demantra 12.2.5.1.
2. Run the Setup.exe .
3. Go Next until it fails.
