Symptoms

When attempting to log into Team Member, the following error occurs:

"The requested page was either not found or you do not have access to it."

STEPS TO DUPLICATE:
-----------------------
The issue can be reproduced at will with the following steps:

  1. Navigate to the Team Member login page
  2. Attempt to log in
  3. Error occurs
