ORA-12505 Error While Applying Patch on Multi-Tier Instance (Doc ID 2180556.1)

Symptoms

On PFT 6.1.1.1, when attempting to apply Patch 21533327 ORA-00904 ON DYNAMIC DRIVER ALLOCATION AFTER UPGRADE TO 6 , the following errors appear in the log files for the DB and WEB tier:

Caused by:
oracle.net.ns.NetException: Listener refused the connection with the following error:
ORA-12505, TNS:listener does not currently know of SID given in connect descriptor

No errors appears when applying the same patch on the App Tier.

The issue can be reproduced at will with the following steps:
1. Run UpdateOFSAAI.sh on DB and Web tier
