Symptoms

When attempting to log into TMS, one of the following errors appear:

ORA-04067: not executed, package "OPA.OPA_USER_SETTING" does not exist

FRM-40735: PRE-FORM trigger raised unhandled exception ORA-04067.

FRM-40735: WHEN-NEW-FORM-INSTANCE trigger raised unhandled exception ORA-06502.

Changes

Database accounts were created using LSH, not TMS.
