IPSAADM User Account Getting Locked While Provisioning Services On Devices (Doc ID 2189695.1)

Symptoms

The Database password was changed for the connection IPSA uses for processes such as the policy server, networkprocessor and system logger.  

This caused the following error to be seen in the networkprocessor logs:

20161003-112332|233|ERROR|p: default-threadpool; w: Idle||||proxy_np|pipsaapp1|DatabaseConnectionPool.makeConnection: SQLException=ORA-28001: the password has expired

20161003-112337|233|DEBUG|p: default-threadpool; w: Idle||||proxy_np|pipsaapp1|DatabaseConnectionPool.makeConnection: count=2

20161003-112337|271|ERROR|p: default-threadpool; w: Idle||||proxy_np|pipsaapp1|DatabaseConnectionPool.makeConnection: SQLException=ORA-28001: the password has expired
