Saving a Comment to the Main EUID Record ends with the error message 'The server encountered unexpected condition which prevented it from fulfilling the request'

ISSUE CLARIFICATION
===================
On Oracle Healthcare Master Person Index (OHMPI) 3.0.0 version,

Add an extension table to the IHE database called SBYN_COMMENT.
Its columns and their datatypes are:
PATIENTID VARCHAR2(20)
COMMENTID NOT NULL VARCHAR2(20)
COMMENTCODE VARCHAR2(32)
COMMENTDATE DATE
COMMENTTEXT VARCHAR2(255)
Via the MIDM, adding a new Comment to the source record is completed successfully.
When trying to add a comment to the main EUID record, the following error occurs:

The issue can be reproduced with the following steps:
1. Add a Comment and Additional Info table;

COMMENTID NOT NULL VARCHAR2(20)
COMMENTCODE VARCHAR2(32)
COMMENTDATE DATE
COMMENTTEXT VARCHAR2(255)

2. Via the front end, add a comment to a source record:
a. In Record details, search for a record and open it.
b. Select Edit EUID.
c. On the right side, the source record is visible.
d. Expand the Comment section.
e. Enter a Comment Code, date and text. Press Save.
f. Press save at the bottom left of the main page. Record is saved;
3. Now add a comment to the Main EUID record on the left:
a. On the left side, expand the Comment section.
b. Enter a Comment Code, date and text. Press Save.
c. Press save at the bottom left of the main page. Error displays:
"We Apologize there is an error in the page. The server encountered unexpected condition which prevented it from fulfilling the request."
