Clarification on OC 5.1 Installation, Accept Connections as a Service (Doc ID 2223540.1)

Goal

The following text is taken from the Oracle® Clinical Installation Guide Release 5.1:

4.1.7 Accept Connections as a Service
Choose to configure the database to accept connections as a service instead of a SID.
Service name requirements for Oracle Clinical include:

-The service name must be less than 15 characters long.
-It must not include the domain.
-It must be all lowercase.

To further clarify what this means i.e. does it mean:
1. Set the init parameter db_domain=null
2. Set the init parameter service_names=<db_name> #without the domain
3. Set the database global name to be the <db_name> #without the domain
4. Configure the tnsnames.ora to use a SERVICE_NAME = <db_name> #without the domain

Or all these, or some combination of these, or maybe something else?
