Collections Process Results in Pending Action Item (Doc ID 2233766.1)

Run collection process on 38K accounts, it is expected with 0 Pending action item, but it results:
1)    38K accounts with Pending Status
2)    run collection process later of the day,  it results 6k accounts with Pending status.
3)    run collection process again, it results 311 Pending action item.
4)    eventually it results 0 Pending action item ...  and why the system behaves this way?
