Attachment Actions Failing Intermittently With Error Code 60086 in Agile PLM

When run a  process extension(PX) to attach a plain text file to the change order, it failed intermittently with error code 60086

Caused by: Error code : 60086
Error message : Call APIException.getRootCause() for details.
Root Cause exception : java.lang.NullPointerException

The issue can be reproduced at will with the following steps:
1. Deploy a PX which is to attach a file
2. Run the PX
3. Sometimes it worked,sometimes it failed.

The issue has the following business impact:
Due to this issue, users cannot attach a file with PX
