Last updated on JUNE 01, 2017

Applies to:

Oracle Clinical - Version 5.1.0 and later
Information in this document applies to any platform.

Symptoms

When a Univariate Discrepancy is created for a question of type Date, the following error occurs:

A system function has encountered an unrecoverable failure.
Please contact your Help Desk.

Subsequently, when attempting to open the CRF again, the following error appears:

The data entry window has encountered an unexpected error and
will shut down. In the problem recurs, please report the following
information for diagnostic purposes:
CRF load was incomplete.

This issue can be reproduced using the following steps:

  1. Create a DCM with at least two questions, one of which is a Date
    Question with DMY format.

  2. Ensure that DE Preferences for the user account in RDC for 'Input Date Format' is European.

  3. In RDC, enter data for one patient with the question Date enter as '28 03 16'. Clicked Save and NO discrepancy is created. Page saves successfully. -- This is correct behavior.

  4. Change your Preferences in RDC for 'Input Date Format' from European to US.

  5. Enter data for another patient with the Date question entered as '28 03 16'. Click Save and a Validation Error Pop-up appears.
    1. Select "Send to Site" for Action and click OK.
    2. Click Save Complete
    3. The discrepancy disappears
  6. Next, click the X to close the Data Entry Window and this message appears:
A system function has encountered an unrecoverable failure.
    Please contact your Help Desk.
  7. After clicking OK on the dialog box, click the X again to close the Data Entry Window and it closes.
Note that the RDC Surround shows that the CRF has a discrepancy.

  8. Click to open the CRF and the following error appears:
The data entry window has encountered an unexpected error and
will shut down. In the problem recurs, please report the following
    information for diagnostic purposes:
    CRF load was incomplete.
  9. Click OK on the error and the Data Entry Window closes.

Additionally, if you try opening the CRF again the CRF will open. However attempting to close the Data Entry Window again results in the same "A system function has encountered..." error again.
