Siteminder SSO User Is Unable to Log Into P6 Web

Primavera P6 EPPM Web configured to use SSO with Siteminder:
After providing valid SSO login credentials, when the initial P6 web username screen appears, the username is NOT populated with the authenticated username.
P6 Professional configured to use SSO with Siteminder:
After providing username and password, error message is received: "Cookie creation failed."
P6 Team Member configured to use SSO with Siteminder:
After providing valid SSO login credentials and attempting to use Team Member, error message received is "The requested page was either not found or you do not have access to it."
