'Calculation Elements' Screen Fails to Open When Defining an ALM Process

Symptoms
In Oracle Financial Services Asset Liability Management (ALM) 8.0.4, when you try to create or edit an ALM Process and go to the "Calculation Elements" screen, the screen does not load. There is nothing listed under the screen as shown below:
Additionally, if you turn on the Internet Explorer debug in Internet Options > Advanced by in-checking "Disable script debugging (Internet Explorer)" and "Disable script debugging (Other)", the following java script error occurs:

Line: 2514
Error: Unable to set property 'disabled' of undefined for null reference
If you select 'Yes' to debug the error further, the error occurs on the following line:
document.getElementById('checkCalOptionValue').disabled = true;
As a result of this error, you cannot complete the definition of the ALM Process.
