OFSAA Login Page Fails to Load with 'Invalid Request' and 'Servlet failed with an Exception'

Oracle Financial Services Analytical Applications (OFSAA)
Symptoms
For an Oracle Financial Services Analytical Applications (OFSAA) 8.0 environment, when you go to the login page, you get the following error:
Invalid request / Error while processing request
Please contact your System Administrator for more information.
When you check the nohup.out from the directory where the web server (ex. Weblogic) is started, you see the following error:
