The Add Activity Button on the Left Pane Isn’t Working As Expected.

When on any screen where you can add an activity at the Policy level, Policy/Role/etc., the Add Activity button on the left Pane (Above Add Comment) .
When the button on the left pane is used, pressing OK or OK Navigate makes the app spin.
Although the activity is properly added, the user receives no indication that it has been and must leave and return to the activity screen to see it pending.
To get rid of the Add Activity dialog box the user must either press Cancel or the "X" button to return to the page they were on.
When either OK or OK navigate is clicked the UI hangs as if it has crashed.
Application should close the dialog and display a notification that the activity has been added or navigate to the activity pane if OK Navigate was clicked.
Steps to Reproduce
1) Navigate to any Policy where an Activity can be added
2) Navigate to any screen on the Policy
3) Using the Add Activity button on the left try to add any activity to the Policy and Click either OK or OK Navigate
4) App will hang until you click cancel or the "X" button
