When attempting to open a Cohort Report, error "ADF_FACES-60096: Server Exception" occurred.

Symptoms

On : 3.1.0.4 version, Dashboards and Reports issues

When attempting to show Cohort Report, the Cohort Explorer shows the following error. In addition, after this error showed up once, the application is in a state, where the same error shows up on almost every click. Even when customer try to log out.

STEPS

The issue can be reproduced at will with the following steps:
1. Login to CE
2. Select Cohort Query > Demographics
  Gender=Female
  Submit
  Patient Count: 20384
  Total Patients in Database: 20384
3. Click " Patient Count: 20384" link.
4. Error "ADF_FACES-60097: For more information, please see the server's error log for an entry beginning with: ADF_FACES-60096: Server Exception during PPR, #11
