Error When Adding Checklist Item to Task - Task Maintenance (open Orders)

MSS users are receiving an error message when manually adding a task and a checklist item to an open order (A.S.R, P.S.R, or EWO) through task maintenance.
Simply adding the new task doesn't seem to have any issues, but the error will sometimes pop up if you try to add a checklist item to that new task; and will then not allow you to save and move on. It doesn't seem to be consistent.
It's occuring randomly and doesn't matter if you add the task as a child or sibling.
The issue can be reproduced at will with the following steps:
1. Open the EWO.
2. Go to Manage Tasks.
3. Right on one of the tasks and Select "Insert Sibling Task". Task gets added .
4-Go to "Tasl Checklist" tab on the same screen and try adding a checklist item .
5-Hit OK and we get the error .
The issue has the following business impact:
Checklist items cannot be added manually to new tasks on open orders.
