'ORA-20005: Application Error Creating Index' Generated When Running CDM Jobs

Symptoms

On OHTR 3.2 version,

When attempting to run the scenarios LOAD_W_EHA_STUDY_D, LOAD_SUBJECT_DIMENSION for the study dimension/subject dimension, the following errors occur:

For the study dimension:

ODI-1228: Task Int_W_EHA_STUDY_D_Full (Integration) fails on the target ORACLE connection ORACLE_TMP_DATAMART.
Caused By: java.sql.SQLException: ORA-20005: Application error creating index W_EHA_STUDY_D_M2 for W_EHA_STUDY_D
ORA-12801: error signaled in parallel query server P01D, instance trc1db01.mayo.edu:cdtrc0011 (1)
ORA-01452: cannot CREATE UNIQUE INDEX; duplicate keys found
ORA-06512: at "CDM.CDM_SCHEMA_MAINT_UTIL", line 98
ORA-06512: at line 2

For the subject dimension:

ODI-1228: Task INT_TMPAPPS_Subject_D_Full (Integration) fails on the target ORACLE connection ORACLE_TMP_DATAMART.
