Provider Group Page Does Not Show All Details and Raises Errors in Log File

Symptoms

Provider group details are not shown correctly in the provider group page:
1. A provider group detail can be found by doing a search on the specific provider code, but the provider is not shown when a search is done without any criteria.
2. The next set button shows some of the same providers as the previous set.
3. The log file shows a large number of error statements every time the page is opened and a search is done.

The expected result is to have all details shown correctly.  The next set button should display another set of provider group details and should not display details shown in the previous set.

No Issues were noticed on this page before
There is no workaround through the UI.  Only a DB query would retrieve the correct details

No errors are shown in the UI.

The log file show repetitions of the following error:

The issue can be reproduced at will with the following steps:
1. Load a provider group with more details than the default number of rows displayed.

Because of this issue,  data and configuration cannot be validated using the UI.
