SA Start Field Activity Creation Not Working (Doc ID 2374810.1)

When attempting to trigger an outbound message to MDM/SOM for the Field Activity, the SA won't save and the FA is not created. Upon attempting to save and start the SA, the application just refreshes and returns to the users home page.

When attempting to manually create an FA, a popup window appears asking if you want to stay on the page or leave it.
