Fields Behavior on Activity Level Requirements Not Working

Fields Behavior on Activity Level Requirements Not Working as expected. We have found a few issues with the behavior of Field elements on Activity Level Requirements they are as follows:
1. Required Tag not working properly - You can leave a field blank and it will let you add it
2. Combo Type=SQL is not populating the options properly - Results from query are not correctly displayed.
3. Field Changes are not being saved when clicking OK

Fields on Activity Level Requirements work as expected

The issue can be reproduced at will with the following steps:
1) Create an Activity Level Requirement
2) Configure a Text, Combo and make another text field required
3) Add this requirement in OIPA
4) You can leave the required field blank and it will still let it be added
4a) Notice the combo box will not correctly display
4b) Insert data into the text field and click okay nothing will be saved

The issue has the following business impact:
Due to this issue, users cannot configure the requirements to work as business requests them.
