Attachment Not Showing In BP Record After Upload

On Unifier : Version 188.8.131.52.
Expected Result: Users should be able to add attachments to business process records and have the attachment show after it is added.
Actual Result: After the user adds the attachment it does not show up in the record until the user does a Save, Close and then re-opens the record.
