On : 11.3 version, Policy Print System (PPS)

ACTUAL BEHAVIOR

Forms List is not pulling the correct form number on the list.

EXPECTED BEHAVIOR
-----------------------
The forms should display with the correct revision date.


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Start a Transaction.
2. Select the forms from the forms list.
3. Check the revision date.

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot issue policies with the wrong revision date.
