C2M Application Menu Has Duplicate Entries and Control Central link missing (Doc ID 2439753.1)

Symptoms

In only one C2M 2.6.0.1 instance for a customer, they notice that the Control Central link in the navigation bar (under the product name in the upper left) is missing, while several menu items are duplicated in the menu fly-out (e.g. Customer Information, Dashboards, Sync Request Outbound).
