' FRM-40508 - ORACLE error: unable to insert record.' when trying to make Active (A) a new question in OC . (Doc ID 2448992.1)

Symptoms

Trying to make active (A) a new question under the OC > Glib > Questions > Questions , it will trow below forms error :

' FRM-40508 - ORACLE error: unable to insert record.'

The real error in the back can be seen by going to  'Help" menu ( In Oracle Clinical) then press on Display Error.

The error in this case was pointing to SAS Label field where this got a maximum limit of 40 characters , where the actual data entered for this field was 42 , therefore the error in the background was :
