Query, Transaction Provision Status Is "Pending"

In a production system it was noticed that the provisioning status of the transactions was always "pending". This included any transactions, even simple ones like positioning changes in the network objects on the map, which does not involve the Network Processor. No logging information gave any information as to the cause.
In the lab environment the transactions state is always "succeeded". Can you let us know what and why the production system does not update the state?
