'Data Entry Error :' appears in the surround when trying to open CRFs. (Doc ID 2468025.1)

Symptoms

1)
RDC Onsite : Unable to open Data Entry windows.

2)
Following error appears in the surround in a 'Patient Casebooks' pop up window : 'Data Entry Error : '

Following error stack (simplified) appears in the 'OpaServer1-de_1.log' on the application server :
