Per Note 1074228.1, under the section titled "2. Check If Notifications are stuck in DB Tables", there are several SQL queries listed to check whether or not notifications are being processed or not. For the last two queries, "Reminders" and "Escalations", it seems to indicate that if there are high numbers returned in the results for these queries then there is an issue.
