Frequent Transaction Feed Management(TFM) Failures

Why are we encountering the following failures with the Transaction Feed Management(TFM) job stream?
F1-FLUSH has been observed to be failing numerous times.
C1-TXNEX also fails several times.
The issues are with following scenarios:
Scenario#1) During server restart and security patching
Scenario#2) After restart of ORMB application
Scenario#3) After restart of threadpool
Scenario#4) When threadpoolworker log size exceeds 1 GB as the logs do not roll out
Scenario#5) When we execute TFM for Sweden first and then execute TFM for other divisions(Norway,Finland)
Scenario#6) When JVM signals the weblogic to shutdown automatically (we suspect system.exit is invoked internally)
