Step insertion indicator seems to corrupt decommission steps

Using step insertion indicator when adding decommission steps seems to result in null parent step.

To reproduce the problem:

  1. Create a planned switch plan
  2. Record a couple of steps
  3. Place the step insertion indicator on the second step
  4. Use the commissioning tool to add a few steps that have both commission and decommission actions. The decommission steps get added first and seem to get null parent steps.

The resulting plan can have problems when executing and even if the users fix the issue, the resulting plan does not print correctly, and does not show correctly in OUA reports.
