Fields Disapear When Switching From Canada To USA In The AddressScreen (Doc ID 2537561.1)

Symptoms

Actual Behavior
-------------------
The fields EffectiveDate and ExpirationDate disapears when the user change the Country fields from Canada to USA or USA to Canada.
In the configuration both fields are configured for addresses for both Counrty. This happens for the AdresstType 14, 15, 16, 17, 18 and 19.

Expected Behavior
------------------------
The fields EffectiveDate and ExpirationDate  should not disapear when the user changes country.

Steps to Reproduce
-------------------------
1-Go to the client
2-Go to the Address Tab
3-Add an address.
4-Select to add an email address.
5- Change the country to USA.
6 EffectiveDate and ExpirationDate disapears.

Business Impact
---------------------
Severe impact. Cannot enter the effective date when creating an address
