OFSAA T2T Completes Successfully But Does Not Load Thousands of Rows - Rows Missing (Doc ID 2539591.1)

Symptoms

In Oracle Financial Services Analytical Applications (OFSAA) 8.0.x, for a T2T Data Load, when attempting to load a large number of data rows from a staging table (ex. STG_CASA) to a data table (ex. FSI_D_CASA) for month-end processing, the T2T completes successfully but not all rows are loaded.  For example, the staging table contains over 400,000 rows but only approximately 200,000 are loaded:

Log Message here 1: Tue 07 May 2019 06:15:19 AM CDT||INFO||No. of records failed for other reasons (INSERT statement preparation, Datatype mismatch etc.) : 0
Log Message here 1: Tue 07 May 2019 06:15:19 AM CDT||INFO||ClsUTableToTableLoad::directLoad, Success, data loaded successfully
Log Message here 1: Tue 07 May 2019 06:15:19 AM CDT||DEBUG||Back from loader : rows loaded ->208178
Log Message here 1: Tue 07 May 2019 06:15:19 AM CDT||DEBUG||Back from loader : rows rejected->0
Log Message here 1: Tue 07 May 2019 06:15:19 AM CDT||DEBUG||Back from loader : Total number of src rows ->208178

You expected over 400,000 rows to load.  There are no error messages in the T2T log from $FIC_DB_HOME/log/t2t or in the FSI_D_CASA$ table.

You do not understand why the rows are missing when there is no error.
