Saving PTA Response Fails with ORA-01400 Error

On : 6.3.0 version, ASR 58
Customer is trying to generate the PTA response and the response is not getting saved.
In the ASR PTA response page , after the data is entered in the admin section, ASR Saved and Validated .It displays "PTA validated successfully" message.
But when users navigate to a different page and come back, the PTA form is blank. The below constraint error can be seen in the AppServerASR58.log file and in the mss.log.
The issue can be reproduced at will with the following steps:
1. Open the PTA response page ASR
2-Fill up the relevant data.
3-Save and validate .
4-user gets "PTA validated successfully" message .
5- Navigate to other page and come back .
6-Check that the filled data has gone .
For an ASR 58 order, users are unable to save the PTA form in GUI.
