Getting "Microsoft VBSCript runtime error '800a000d'" when attempting to Patient & Activites tab

Patient Tab:

"Microsoft VBScript runtime error '800a000d'
Type mismatch: '[string: ""]'

Activities Tab:

"Microsoft VBScript runtime error '800a000d'
Type mismatch: 'PrimaryContact'
/CaseForm/Activities/CF_ContactLog_inc.asp, line 415

Steps to Repeat:
1. Open the case
2. Go to Patient or Activities tab
3. Observe that it throws the error
