In a back-end Mail store, IMAP commands such as the one shown below can prevent mail delivery:
From this example, it seems plausible that if an account with 3 times as many messages took 3 times as long, the client might timeout and try again, resulting in several threads waiting for the lock to repeat the same thing.
Obtaining a pstack on the IMAPD process a few times during the above transaction showed that it seemed to be in the config code constantly.
