Symptoms
Order creation fails with the following error in the OSM server logs:
ORA-00001: unique constraint (<schema name>.XPKOM_ORDER_FLOW_COORDINATOR) violated
This is an intermittent issue, and occurs suddenly for certain orders.
Changes
Changes were made in OSM 7.2.4.1.11 which introduced this issue
