Bill Segment Has 2 Headers Instead of 1 (Doc ID 2605960.1)

Symptoms

On RMB v2.8.0.0.0, C1-FIBCR- Creates Billable Charges with Charge Line and SQI which generated two Bill Segment Headers.

Scenario:
Add a member with Medical Coverage, Employee Only Coverage Tier and Effective Date 5/1/2019.
Group Customer has 2 bill groups (Active and COBRA) and 2 plans (Medical and Dental).

ACTUAL BEHAVIOR

Bill shows two $90 charges for the coverage periods being billed, but the underlying data appears to be incorrect:

- After running C1-REPC1, C1-REPC2 and C1-FIBCR the Billable Charge has both Billable Charge Line and SQI
- When we create a bill it creates two Bill Segment Headers for one bill segment.

EXPECTED BEHAVIOR

C1-FIBCR should create billable charge with SQI only and when we create a bill it should have only one Bill Segment Header for bill segment.

Member should be set up under Active Bill Groups (Account) Medical Plan with a Billable Charge for $90 with one Invoice for 2 coverage periods:
5/1/2019 – 5/31/2019 with a $90 charge
6/1/2019 – 6/30/2019 with a $90 charge

STEPS

The issue can be reproduced at will with the following steps:
1. Install Membership: 05-01-2019 to 04-30-2020
2. Run C1-REPC1
3. Run C1-REPC2
4. Run C1-FIBCR
5. Generate a Bill

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Generating multiple bill segment headers may be confusing to a user viewing the data. This may also cause issues with other processes that look at the Bill Segment.
