MWM Mobile App Is Not Displaying Questionnaire With Only 1 Question (Doc ID 2666217.1)

The MWM Mobile app is not displaying Questionnaire (Service History) when it has only 1 question.
The questionnaire section in MWM displays the questionnaire perfectly however when it gets to mobile app it doesn't display the question

Note that its on an iOS device hence the device logs cant be provided.
