All the customers that are failing have been found to have 9 digit zip codes

In Regions and Zones - Location tab -- We see the location mapping as
United States US
United States US New York NY
United States US 9 digit zip 9 digit zip

In sourcing we have
Instance-Region sourcing setup where Region is United States - Nebraska
