Transformation Failure Caused by Duplicates from a Custom Program

Symptoms
While working on a Transformation, it was needed to update a custom program. For the installed Clinical data model, the latest table descriptor was added while updating the latest custom program in LSH. After that attached the latest version of the custom program to the Transformation, but the custom program became non-installable.
Tried adding all the table descriptors several times (by deleting the tables with the previous "INCOMPLETE" status) in the custom program in LSH. But again the same thing is happening, that is at the time of implementation it shows "COMPLETE" but after Validation and installation the status changes to "INCOMPLETE" again. Due to this the Transformation installation is failing.
