Database User Fails to Convert to LDAP User with Warning: "Following users were not updated because they have duplicate user IDs"

Symptoms
The database authenticated user is not converting to LDAP user even after refreshing LDAP information in the database and running Refresh Users from LDAP directory as explained in Note:568607.1
Below warning appears when performing LDAP sync:

Warning: Following users were not updated because they have duplicate user IDs: {username} ({LDAP ID})
Steps
The issue can be reproduced at will with the following steps:
- Log into Java Client as admin user, and confirm that below users appear under LDAP preview screen
{username} - Log into sqlplus as agile database user, and run below sqls:
- See GUID and AUTH_SRC is not populated for these users
