Symptoms
In Google Chrome, when attempting to create a new Form in Forms Designer, you reach Step 4 where you select the needed table. However, when you click on "Next", nothing happens. The screen stops working and does not advance to the next step.
You expect the next screen "Fields Selection Screen" to load after clicking on Next.
You can create the Form successfully if you use Internet Explorer.
