CM Reports Error "could not open pcp context to DM" (Doc ID 2806002.1)

Scenario:

After a customer deployed a new custom code the following errors started to appear in the Connection Manager pinlog frequently
There are no errors reported by any other components which could be matched by time or type.
The connections between components (host ip, port number etc) were not changed.
There appears to be no functional impact to any of the operations.
This is a multi-schema system with one primary and two secondary schemas
