Case Justifications For User-defined Fields Are Being Lost

Symptoms
Case justifications for user-defined fields are sometimes being lost while saving or navigating.
User defined 8 date fields to be used on the General page. The justification fields are used to save reference numbers.
This is how those fields appear in the UI:
User enters a justification.
After user enters and saves the case, the values disappear and the dot turns from green to amber.
It was also observed at the database level that the hidden values have primary_seq_num =1, whereas the unhidden values have primary_seq_num > 1,
apparently equal to one of the case_Event_assess.seq_num values.
Replication Steps:
1)Un-hide one or more user-defined date fields on General tab.
2)Bookin a case and enter values for the UD-date fields along with justification values (dots turn green), save case.
3)Enter a second event, save case.
4)Enter causality assessments for both events.
5)Return to General tab, green dots are now turned to amber, and justifications are gone.
