F2T And T2T Blank Source Entity Map Selector Screen After Applying 8.1.1.2.0 (Doc ID 2822520.1)

Symptoms

After applying AAI 8.1.1.2.0 <Patch 33071689>: OFSAA 8.1.1.0.0 AAI ML RELEASE #2 (8.1.1.2.0), the Data Mapping T2T and F2T screens now show blank screen for Source Entity selection.

There appear to be errors in the Tomcat catalina.out log:

The issue can be reproduced at will with the following steps:
1. Common Object Maintenance -> Data Management Tools -> Data Mapping
2. Select existing T2T and Edit
3. Click Map Button

Screen appears to be broken, nothing appears.
