Not Able To Promote Any Workflows Getting the Following Error in Change Status Window “Ora-00001: Unique Constraint (AGILE.SIGNOFF_PK) Violated (Doc ID 2883550.1)

Symptoms

When attempting to promote/demote the Change order workflow from current status the following error occurs.

ERROR
”ORA-00001: unique constraint (AGILE.SIGNOFF_PK) violated”

STEPS
The issue can be reproduced at will with the following steps:
1. Open a browser and Launch Agile PLM Web Client using url http://localhost:port/Agile/PLMServlet
2. Log into Agile PLM Web Client
3. Open any Change Order Objects, Go to Workflow and Try to promote/demote from current status.
4. In Change Status Window : Getting the following error ”ORA-00001: unique constraint (AGILE.SIGNOFF_PK) violated”
