'ORA-03114' / 'FRM-40735' Errors Appear When Using The Oracle Clinical Application.

Symptoms
1)
Oracle Clinical users are encountering errors when using the application for over 20 minutes.
2)
Following errors appear when the user tries to use the application:
- 'FRM-40733: PL/SQL Built-in DBMS_ERROR_TEXT failed' and/or
- 'FRM-40512: ORACLE error: unable to issue SAVEPOINT command'
Eventually after clicking on [OK] many times, the 'FRM-40735: WHEN-VALIDATE-ITEM trigger raised unhandled exception ORA-03114' error appears on the status bar i.e.

After encountering the above errors user has to kill the 'Java(TM) Web Launcher' process (jp2launcher.exe) via Task Manager to close down the Oracle Clinical application.

Following were checked and ruled out:
i.
Below forms setup would not cause a timeout:
- FORMS_TIMEOUT=10
- Heartbeat=7
ii.
Database profile associated with user has UNLIMITED for below:
Parameter will disconnect a session whose connect time exceed the value for connect_time.The connect_time is expressed in minutes.
IDLE_TIME:
Permitted periods of continuous inactive time during a session (minutes).
