Symptoms
On RMB v3.1.0.0.0, Unique constraint error for CI_MEMBERSHIP_PROCESS record.
Getting Unique constraint error for CI_MEMBERSHIP_PROCESS record when membership gets terminated for second time.

The issue can be reproduced at will with the following steps:
1. Create a Membership and make it Active
2. Terminate the Membership
3. Reactivate the Membership
4. Terminate the Membership and it gives System Error
Log shows:
java.sql.SQLIntegrityConstraintViolationException: ORA-00001: unique constraint (CISADM.SYS_C00139500) violated
Application should terminate the membership instead.
