AE Form is showing “Not Verified” under the Monitor tab while AE Form is Verified under Subject tab (Doc ID 2901394.1)

AE form on subject tab is source verified while source verification shows un-verified when viewing monitor tab.  Source Verification status does not match verification status between monitor tab and subject tab.

Due to <Bug 29093626>, when a CRF in a visit had been designated as Dynamic and Common, the Form placeholder record that is intended to keep track if a CRF had been triggered for a particular patient may have its STATE incorrectly set to 1 (STARTED state). When such incorrect Form records exist in the trial, there will be duplicate CRF shown in the Monitor page, one for the internal InForm’s Common visit and the other for the Context visit where the visit is triggered. This will cause confusion to the end user.
