Unable to Add Attachments from Web Client, Status Shows "In Progress" But Never Completes, Error: "The system cannot find the path specified" in File Manager Log

Symptoms
Unable to upload attachments from the Web Client. The status shows "In Progress" but it never completes. File Manager is running on Windows server machine.
Below errors can be seen in the File Manager log:
ERROR com.agile.webfs.components.fileserver.FileServerDispatcher - Error
com.agile.webfs.components.vault.client.VaultException: \{path}\XXX\XXX\XXX\{file} (The system cannot find the path specified)
<FileServerDispatcher:ERROR> Error
com.agile.webfs.components.vault.client.VaultException: \{path}\XXX\XXX\XXX\{file} (Access is denied)
When attempting to upload file from Java Client, below error is seen:
\{path}\XXX\XXX\XXX\{file} (The system cannot find the path specified)
The issue can be reproduced at will with the following steps:
- Log into Web Client, and click My Settings, and go to Attachments tab.
- Click Add. File Upload Selector window shows up.
- Click Browse for files, and select the file, and click Upload.
- File Uploader Manager window shows up. See the status shows "In Progress" for the file being uploaded, and never completes.
- Check File Manager log (agilefm-stdout.YYYY-MM-DD.log) under %Agile_Home%\FileManager\logs, and see the error.
- Login to Java Client, and click View/Edit Current User, and go to Attachment tab.
- Click Add File(s) > Files, Browse for Files, and select the file, and Upload. See the error message pops up.
